Screening Questions
Face Masks are required.
Your temperature will be taken prior to visiting.
If you to answer “yes” to any of the following symptoms or questions, you will be asked to visit another day.
Covid-19 Related Symptoms:
Fever
Dry Cough
Sore Throat
Shortness of Breath
Have you been in contact with anyone positive of Covid-19 within the previous 14 days?

Outdoor Visit Guidelines
We are balancing the position of safety and involvement. Please understand that we absolutely want to open the doors and return to “normal.” However, until we have a stronger handle on the situation, we will have restrictions in place that protects the health and safety of your loved ones and our community.
On October 5th, we will begin limited outdoor visits – by reservation only.
All Visits are by Reservation Only. Please reserve a visit at www.mdvcontact.com
Understandably there is a very high demand for families to see their loved one.
To give all families an opportunity, at this time we are asking that to please only reserve one outdoor visit per resident per week.
No Physical Contact
Face Masks are required (for both visitors and residents)
Location: Main Entrance behind the Flag Poles and Fountains. There will be a designated area upon arrival.
All visitors will be screened prior to visiting
Maximum of 2 visitors per visit
Adults only – no one under the age of 18 at this time
Visits are limited to 15 minutes
Outdoor visits are subject to satisfactory weather that does not pose a risk to any residents, staff, or visitor
No food
Bottled Water will be available
